Adopting and maintaining healthy lifestyles can be challenging enough on your own - but it can be especially challenging to encourage your child to follow these habits. One of the best keys is to create a home environment that makes the healthy choices the easier choice.

The Family Nutrition and Physical Activity (FNPA) screening tool is a behaviorally based assessment that is designed to help parents evaluate their home environment and practices related to healthy living. The FNPA will specifically help parents to assess the extent to which they are shaping and influencing lifestyle habits in their children. There are 20 questions that evaluate difference aspects of the home environment and when you complete them you will receive specific information and resources that can help to promote healthy lifestyles in your child.



 
The Family Nutrition and Physical Activity (FNPA) survey tool is designed to help parents evaluate behaviors and environments related to children’s physical activity and nutrition behaviors.  The original version of the FNPA tool was developed in partnership with the Des Moines Independent School District back in 2005 and we need your help to evaluate an updated version. Your efforts will help to refine the tool but your participation is completely voluntary.
 
If you decide to participate in this research study, you will be asked to complete an online version of the 20 item FNPA instrument along with some basic demographic questions.  The FNPA contains questions about your child’s physical activity, nutrition, screen time and sleep habits as well as items about your home environment and policies related to your child’s behavior.  The questionnaire should take approximately 10-15 minutes to complete and your responses to the questions will be linked to your child’s weight and fitness data (collected through their physical education classes) using their Student ID.  No names are tracked in the software.
 
There are no foreseen risks associated with your participation in this study, and while there are no direct benefits to you from filling out the survey, you may find useful information regarding helping children be healthy on the Kids Eat Right website where the survey is housed. Your participation is completely voluntary and you can skip any question you don’t want to complete or stop participating at any time.  You may refuse to participate without any consequences to you or your child.  Information you provide will only be used to help evaluate the FNPA tool and your responses will be kept completely confidential.  The results of this research study may be published but your information will not be shared with the school district or with anyone else not involved in the study.
 
Your name is not stored on any file so your responses are confidential.  Only researchers involved in the project will see your responses; however, federal government regulatory agencies, auditing departments of Iowa State University, the Institutional Review Board (a committee that reviews and approves human subject research studies) may inspect and/or copy study records for quality assurance and data analysis. These records may contain private information.
